- Checklist form: name, email, agency, role
- Waitlist form: name, email, agency, role
- Calendar booking: name, email, and any context you share when scheduling a consultation through the Google Calendar embed
We do not collect Protected Health Information (PHI). Do not submit patient names, dates of birth, medical record numbers, or any other identifiable patient information through any form on this site. We are not a covered entity or business associate under HIPAA.
2. How We Use Your Information
We use the information you provide to:
- Respond to your inquiry or fulfill your request (e.g., send you the free checklist)
- Communicate with you about services you’ve expressed interest in
- Send occasional updates about new content, services, or course launches (you can opt out at any time)

- GitHub Pages (hosting): receives your IP address and request data
- Google Forms (form submissions): stores your form responses on Google’s infrastructure
- Google Workspace Calendar (scheduling): collects booking details you provide

The site itself does not set cookies or use local storage. Third-party services listed in Section 3 (Google Forms, Google Workspace Calendar) may set their own cookies when you interact with their embeds. You can control cookies through your browser settings or by enabling Do Not Track.

We retain form submissions and email correspondence for as long as needed to provide our services and comply with legal obligations. You can request deletion of your data at any time (see Section 7).
